Go for it

OK, now you're prepared.  You have your evaluation, and you've documented your contributions and your results over the past year.  You know where your skills and responsibilities fit on the company career ladder, and you have some idea of the range of salaries available to people in your position (or the position you hope to move to).  And you know what people are making at similarly-sized companies in your location.  Also, you're aware if there are special circumstances which might limit your company's ability to give you more money.

Go for it.  Sit down with your boss and lay out your case.  Do it without emotion, and certainly without threats.  And remember, it's about what you deserve based on your contributions relative to what others are making.  It has nothing to with what you 'want' or 'need'.

Also, focus on the results you achieved over the last year, not on your skills.  As I said earlier, your skills are your potential, and the company won't reward you for potential.  They will reward you for what you actually accomplished over the year (regardless of your skills).  If you've got skills that you aren't able to use to produce results, you may be in the wrong job.  But don't expect your boss to pay you for skills you aren't using.
